IMPLANT CARD

This page provides an implant card template and related instructions. The implant card is intended to support traceability of the implanted device (e.g., UDI, batch/lot) and to help the patient and healthcare providers identify the implant.

Instruction for completion

  1. Patient name or patient ID. To be completed by the healthcare institution/provider according to local procedures.
  2. Date of implantation. To be completed by the healthcare institution/provider.
  3. Name and address of the implanting healthcare institution/provider. To be completed by the healthcare institution/provider.
Note: The implant card should be provided to the patient. The device label (UDI) should be attached in the dedicated area, when applicable.
